What you get with a subscription

The examples come from a case in the app: Laura, 53, pain under the right ribs since last night.

Gallbladder ultrasound: distended gallbladder with a stone in the neck
Gallbladder and biliary tract ultrasound. Image: Turki, Ahmed et al. (Baghdad, Iraq), CC BY 4.0

Exam images, with the explanation

Biliary tract and gallbladder: distended gallbladder with a stone impacted in the neck, a thickened wall, pericholecystic fluid and a positive sonographic Murphy's sign.

Teaching explanation

The stone is a hyperechoic (white) image inside the gallbladder, with a black acoustic shadow behind it, because the ultrasound cannot pass through it. Here it does not move when the patient changes position: it is impacted in the neck and blocks emptying, and indeed the gallbladder is distended.

Questions in your own words

Have you taken anything for the pain?

Keywords

if I take a deep breath it gets worse.

No, nothing. I hoped it would go away on its own, like the other times.

Does anything help?

Keywords

I'm not sure I understood what you mean.

Nothing. Breathing deeply makes it even worse.

The keyword answers are the real ones from the app. The subscription ones are an example: the model answers in its own words every time.

If you get it wrong, you can try again

Your answer: Acute pancreatitis Wrong

Review your notes and tests, then try again. You can also see the solution now.

Frequently asked questions

All questions
Can I cancel whenever I want?

Subscriptions cannot be activated yet, so the terms are not set yet. They will appear on the Subscription page, together with the price, before it becomes possible to subscribe.

Are my progress and scores saved?

Yes, if you sign in with Google: diagnoses, scores and valid days for the bonus appear on the Progress page, in the account menu, together with your latest finished cases. Without an account nothing is saved beyond the case in progress.

Does it work on a phone?

The interview, physical examination, laboratory and diagnosis sections do. The DICOM viewer for ultrasound, MRI and CT needs a larger screen: on a phone you can scroll through the images, but assessing them is awkward.

Are the cases real? Have they been validated?

No. They are written for teaching purposes, do not describe real patients and have not been clinically validated. They serve to practise diagnostic reasoning, not to make care decisions. The same applies to the patient's answers generated by the language model, which may contain inaccuracies.
